    In relation to the types of palate expander, when we expand the upper jaw or palate it is better to use a fixed palate expander for various reasons:

    • As it is fixed, the expansion is more skeletal as it works 24 hours a day and therefore the expansion is more stable over time.
    • The treatment time is reduced to 10-12 months.
    • As it is fixed, it does not depend on the patient’s cooperation. There is no possible loss of the expander.

    The discomfort when a fixed palate expander is cemented in normally lasts 2 days. The only discomfort is that it is somewhat difficult to eat and speak due to the lack of habit of having the appliance in the mouth. However, in two days the patients forget that they are wearing the expander and get used to it perfectly.

    You should avoid all foods that get stuck to the appliance, such as chewing gum, because if they get stuck to the palate expander, they are difficult to remove and could also cause the expander to become detached. If the expander becomes detached, you should visit the clinic as soon as possible so that it can be replaced.

    It must be cleaned with a conventional toothbrush. In addition, it is necessary to apply a jet of pressurised water, which you can do in the shower, with syringes, or with a Waterpik type device that expels pressurised water.

    The fixed palate expander or maxillary disjunctor can be fitted up to puberty (in girls up to 13-14 years of age and in boys 14-15 years of age). The exact time depends on the skeletal maturation of each patient, as the palate expanders separates the maxillary suture in the midline and this can only be done while the patient is growing. As soon as the skeletal sutures have fused, we can no longer expand the maxilla using the maxillary expander alone and other procedures must be used.

    When the patient is a young adult who has passed puberty, the sutures are already fused, so it is not possible to separate the suture from the maxilla using the palatal expander alone as it will not be strong enough to separate the suture. For orthodontic treatment between puberty and up to the age of 20 we can expand the jaw in a predictable way with the MARPE technique which consists of a combination of the palate expander and micro-screws. These micro-screws offer the possibility of applying greater force and being able to expand the palate in a predictable way as before puberty.

    • The expansion is used to make more space for the eruption of the permanent teeth.
